You’ve been watching your diet and following your usual exercise routine. But your pants seem a little tight and, sure enough, the scale shows that you’ve gained five pounds in the past month.
What’s going on?
This may be hard to swallow, but a medication your doctor prescribed could be to blame. Certain prescription drugs used to treat mood disorders, seizures, migraines, diabetes, and even high blood pressure can cause weight gain - sometimes 10 pounds a month. Some steroids, hormone replacement therapy, and oral contraceptives can also cause unwanted pounds to creep up on you.
But even if you suspect a prescription medication is causing weight gain, never stop taking the drug without consulting your doctor, experts stress.
“Stopping some of these medications on your own can have very serious consequences,” says Louis Aronne, MD, director of the Comprehensive Weight Control Program in New York City and president of the North American Association for the Study of Obesity. “It has to be done very carefully.”
Madelyn H. Fernstrom, PhD, director of the Weight Management Center at the University of Pittsburgh Medical Center, agrees. Even if a medication causes weight gain, “an extra 10 pounds may be worth the trade-off of what that medication is doing for your overall health,” she says.
Common Offenders That Can Cause Weight Gain
While no one knows exactly how many prescription drugs can cause weight gain, experts estimate the list includes more than 50 common medications.
Steroids such as prednisone, older antidepressants such as Elavil and Tofranil, and second-generation antipsychotics like Zyprexa are the biggest — and most recognized — promoters of weight gain, Fernstrom says.
Some other common offenders, says Fernstrom, include the antidepressants Paxil and Zoloft, the antiseizure medication Depakote, diabetes drugs like Diabeta and Diabinese, and the high blood pressure drugs Cardura and Inderal. Heartburn drugs like Nexium and Prevacid may also cause drug-induced weight gain.
Fernstrom tells WebMD that the medication-associated weight gain can be modest — or as much as 30 pounds over several months.
“And in some cases, it is unrelated to the action of the drug itself,” she adds. “For example, if an antidepressant makes people feel better, their appetite may be restored and they eat more.”
Making matters more complicated is that some drugs, like Prevacid and Nexium, can cause weight gain in some people and weight loss in others.
“Not all drugs have the same side effects for all people,” she says. “You have to work with your doctor to find the drug that’s right for you.”
Alternatives That Don’t Cause Weight Gain
Aronne says he warns against putting too much stock in a list of specific drugs that cause weight gain.
“What you need to know,” he tells WebMD, “is that certain types of drugs can cause weight gain.” But in almost every case, the doctor will be able to switch you to another medication that has the same desirable effects but which will not cause weight gain and may even help you to shed a few pounds, he says.

This is one of those herbs that are good for a great many disorders. Among them is cystitis, but I don’t think I’d try to treat that condition with a home remedy. I’d see my doctor for that. But burdock is good for a wide variety of disorders, and as a diuretic, it would be helpful for high blood pressure. However, you’d need to see a doctor for that because it would depend on what medication he prescribes. Camphor- Camphor is one of my favorite remedies as it was given to me as a child by my grandmother if I had a cold. She rubbed it on my chest and back, and since it’s already a treatment for rheumatism, I got the benefit of a rubbing for that. Grandmother always carried a small camphor container in her purse, and, as women did in those days, would sniff it if feeling faint. The caution is not to take too many sniffs of the camphor because of its powerful effects. But the camphor oil is definitely recommended for rheumatic joints. Rub the oil on the affected joint and the pain should begin to clear up quickly. Centaury - A traditional remedy for gout or rheumatism, centaury flowers are what make the medication. Centaury also can be used to treat heartburnit’s used in some alcoholic drinks as a bitter flavor and tonic effectand for reducing fever. The Tendency to Develop Diabetes in Depressed Patients Isn’t Connected With the Particular Medicine They Take

It is always fascinating to watch how the media pick up on a story from the world of medical research. No matter how well balanced and scientifically neutral the source material may be, it seems that news must always be more sensational. Only bad news is good news to sell newspapers and keep the advertisers happy. Take a headline, Toothpaste gives you cancer! Panic not. I just made that up, but you get the idea.

So, as responsible journalists, how should we approach an article in the January edition of Diabetes Research & Clinical Practice? Well, I jest, of course. Actually approaching it in the first place is difficult because this is hardly a hot-from-the-presses must-read magazine. You actually have to be searching for research with a magnifying glass. Anyway, the authors beat the grasses with a stick in the Canadian province of Saskatchewan to see what snakes would come out. Brown, Mujumdar and Johnson had the data on the incidence of type 2 diabetes from 1st January, 1991 to 31st December, 2001 among patients who were depressed. They also had the medical records showing the medications prescribed. The majority were taking either or both a Selective Serotonin Reuptake Inhibitors (SSRI) like Zoloft or a tricyclic antidepressant (TCA) like Aventyl or Elavil.

So let me start with two relevant findings:

if you live in Saskatchewan and you are depressed (no cause and effect here, of course), you are 30% more likely to develop type 2 diabetes than someone who is not depressed; and

if you take two medications at the same time, this doubles the likelihood that you will develop diabetes (about 10% of those whose records were available did take two).

The TCAs were first used in the 1950s but, as the newer medications including the SSRIs have come on to the markets, the TCAs have been increasingly phased out because they are considered more likely to cause side effects. Thus, in the period covered by the research, it would not be surprising that patients should be taking both. Either they would be phasing out, say, Moxdil in favor of the newer Zoloft (introduced in 1991) or they were adopting a belt-and-braces approach and combining the old with the new, hoping for the best result. Today, it should be quite unusual to find a significant number of people taking Zoloft and one of the TCAs. But, if you are one of these people, you should stop taking the TCA right now.

So let us be absolutely clear on the message here. There is no link in this research between Zoloft and diabetes. Indeed, in the literature, there is a considerable body of evidence to show that the link is more certainly between a depressive illness and diabetes, cardiovascular diseases, etc. The reason is scientific confirmation of a common sense or intuitive truth. That people who are depressed do not look after themselves with the same care and attention as healthy people. They lack the motivation to exercise. They eat comfort food and put on weight. This increases the risk factors for diabetes and heart attacks.

So, there is no need to change from Zoloft on the basis of this research, but you still have to watch out for Emperor Zoloft and his hungry-for-hamsters evil empire.

Migraine Headaches and TMJ Syndrome

Headache and TMJ syndrome may go together if you have chronic recurring headaches. While wildly over diagnosed in the 1970’s recently newer imaging techniques, such as MRI’s, have allowed for a better look at this facial joint that causes so much pain.

TMJ or temporomandibular joint syndrome as the disorder is commonly called is poorly understood at the present time in terms of how it is caused. While many patients have bruxism or grinding of the teeth at night, others may have had trauma, osteoarthritis, or rheumatoid arthritis. Many times the cause is unknown. For those of you with migraine, the constant clenching of the jaw causes muscle spasms in the temples. This then leads to a migraine.

The symptoms of TMJ may be any or all of the following: pain or tenderness in jaw and/or around the ear, difficulty chewing, a clicking sound when jaw is opened, spasms causing the jaw to “lock” when open or closing, uncomfortable or uneven bite, and finally, headache!

Testing for TMJ

Diagnosing TMJ involves a good history of how you sleep, grinding habits during the day and general tension. An exam of the jaw should involve palpation of the joint both inside and outside of the mouth. Imaging of your jaw may be needed to find out how much damage is in the joint. X-rays, a CT of the jaw or even an MRI would assist the doctor.
Once diagnosed you should be referred to a dentist who specializes in TMJ syndrome.

Treatment of TMJ

Treatment of TMJ may greatly reduce your headaches. But like anything else, a multi-factorial approach to the problem will yield the best results for you. These are the most common treatments for TMJ sufferers who also have migraine.

Overuse of jaw muscles. This is mostly a lifestyle change. You must be willing to recognize when you are grinding your teeth during the day and learn to relax the jaw. Also, try not to eat foods that are difficult to chew until you heal a bit more.

Appliances .Depending on your particular situation, the dentist may make a bite plate or splint to help align your teeth at night. A night guard appliance could also be used to help prevent grinding.

Medications. Various medications may ease your pain and thereby reduce your headaches. A short course of steroids will reduce inflammation, or if the condition is not severe an anti-inflammatory could be used. Spasming of jaw muscles could contribute to the problem, and the ideal medication for this is Elavil.

Dental Repairs. Your dentist may recommend dental work that may not make much sense to you at first. However, correcting bite, repairing teeth and filing down tooth edges that aren’t meeting correctly will all serve to decrease TMJ.

Surgery. This is a final option and is reserved for cases where the above options have failed. In this case the joint is so badly damaged that joint reconstruction or replacement may be recommended. And again..your headache and TMJ may be reduced!

If in doubt, see your dentist to begin with and ask about testing for TMJ. Discuss this with your headache specialist also as they are very familiar with the condition. If you do not see a headache specialist, your primary care physician should be able to help.
